Registration and Attestation

Our System

The Centers for Medicare and Medicaid Services (CMS) has created a web system for the Medicare and Medicaid Electronic Health Record (EHR) Incentive Programs. Those providers who are interested in taking part in the program will have to use this system to register and submit their intent to participate. Registration does not mean that the providers are committing to the program; it only means that they are interested in participating. Registration for Medicaid and Medicare providers should be completed at the CMS EHR Incentive Program website.

The attestation step follows the registration process. Attestation is the step of submitting the information required to qualify for the Medicare or Medicaid EHR incentives. Attestation for Medicare providers and hospitals should be completed on the CMS EHR Incentive program website. Attestation for Medicaid providers and hospitals should be completed on the state’s attestation website.
