About Us

Accountable Care Organizations (ACOs) “are groups of doctors, hospitals and other health care providers, who come together voluntarily to give coordinated high-quality care to their Medicare patients.”* When the care they provide saves money for Medicare, they receive a share of the savings. By joining an ACO, providers also can participate in a more population-based approach to care that promotes patients receiving the right care at the right time.

For the past four years, NJII has provided Medicare shared-savings support to several of the largest ACOs in New Jersey. We are a proven and trusted team that aggregates our clients’ quality data and prepares it for submission to the Centers for Medicare and Medicaid Services (CMS). NJII also provides a team of highly trained nurses to abstract quality measure values from both Electronic Health Record and paper or scanned charts to completely meet CMS reporting requirements.

  • Data capture via Claim and Claims Line Feed (CCLF), electronic medical record data and manual nurse abstraction
  • Group Practice Reporting Option (GPRO) submission

  • Physician and staff education on measures and workflows for Electronic Medical Record data collection
  • Abstraction Progress Reports that track critical elements such as beneficiary ranking and skips so an ACO can monitor its performance at any point in the GPRO audit process.

The team utilizes our innovative “capture” software tool to ensure that all quality values are collected and entered accurately.
